Antifungal susceptibility testing (AST) is crucial in clinical settings to guide appropriate therapy. Nevertheless, discrepancies between treatment response and some results still persist, particularly in detecting resistance to amphotericin B (AMB) in Clavispora (Candida) lusitaniae. This study aimed to assess the susceptibility patterns of 48 recent isolates of C. lusitaniae to 9 antifungal agents and explore the feasibility of using a CLSI reference-based method to identify AMB resistance. Microdilution techniques revealed a wide range of minimal inhibitory concentration (MIC) values for azole antifungals, while echinocandins and AMB exhibited a narrow range of MIC values, with all strains considered wild-type for the tested polyene and echinocandins. However, when agar diffusion (ellipsometry) was employed for AST, certain strains displayed colonies within the inhibition ellipse, indicating potential resistance. Interestingly, these strains did not respond to AMB treatment and were isolated during AMB treatment (breakthrough). Moreover, the evaluation of AMB minimum fungicidal concentrations (MFCs) indicated that only the strains with colonies inside the ellipse had MFC/MIC ratios ≥ 4, suggesting reduced fungicidal activity. In conclusion, this study confirms the effectiveness of ellipsometry with RPMI-1640 2% glucose agar for detecting AMB resistance in C. lusitaniae. Additionally, the proposed approach of culturing "clear" wells in the microdilution method can aid in uncovering resistant strains. The findings highlight the importance of appropriate AST methods to guide effective treatment strategies for deep-seated candidiasis caused by C. lusitaniae. Further collaborative studies are warranted to validate these findings and improve the detection of AMB clinical resistance.

Matrix-assisted laser desorption/ionisation-time-of-flight mass spectrometry (MALDI-TOF MS) seems to be an option for improving identification. AIMS: To develop a database (DB) for the identification of dermatophytes with MALDI-TOF MS, including 32 isolates from the Red de Micología de la Ciudad Autónoma de Buenos Aires [Mycology Network of the Autonomous City of Buenos Aires] (RMCABA) and one reference isolate (RMCABA DB), and evaluate its performance when added to the DB from the supplier, Bruker (Bruker DB). METHODS: All the isolates in the RMCABA DB were identified based on morphology and sequencing. To evaluate the performance of the extended DB (Bruker DB plus RMCABA DB), 136 clinical isolates were included. RESULTS: The percentages of identification at the species level increased from 45% to 88%, but the identification at the genus level decreased from 23% to 7%. CONCLUSIONS: MALDI-TOF MS yielded better performance in the identification of dermatophytes after including the RMCABA DB, which encompassed local isolates.

Antecedentes: La identificación de dermatofitos se realiza, usualmente, a través del análisis morfológico, pero puede verse dificultada por la aparición de nuevas especies y complejos de especies y, en algunos aislamientos, por la falta de elementos de fructificación. Matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS) aparece como una alternativa para mejorar la identificación. Objetivos: Construir una base de datos (BaDa) para la identificación de dermatofitos mediante MALDI-TOF MS, con 32 aislamientos de la Red de Micología de la Ciudad Autónoma de Buenos Aires (RMCABA) y una cepa de referencia (BaDa RMCABA), y evaluar su desempeño al incorporarla a la BaDa del proveedor Bruker (BaDa Bruker). Métodos: Todos los aislamientos incorporados a la BaDa RMCABA se identificaron por morfología y secuenciación. Para la evaluación de la BaDa ampliada (BaDa Bruker más BaDa RMCABA) se incluyeron 136 aislamientos clínicos. Resultados: El porcentaje de identificación a nivel de especie se incrementó del 45 al 88%, mientras que la identificación a nivel de género disminuyó del 23 al 7%. Conclusiones: Con la incorporación de la BaDa RMCABA, que contó con aislamientos regionales, se observó un mejor desempeño en la identificación de dermatofitos por MALDI-TOF MS.(AU)

Information on paracoccidioidomycosis (PCM) in Argentina is fragmented and has historically been based on estimates, supported only by a series of a few reported cases. Considering the lack of global information, a national multicentric study in order to carry out a more comprehensive analysis was warranted. We present a data analysis including demographic and clinical aspects of a historical series of 466 cases recorded over 10 years (2012-2021). Patients were aged from 1 to 89 years. The general male: female (M:F) ratio was 9.5:1 with significant variation according to the age group. Interestingly, the age range 21-30 shows an M:F ratio of 2:1. Most of the cases (86%) were registered in northeast Argentina (NEA), showing hyperendemic areas in Chaco province with more than 2 cases per 10,000 inhabitants. The chronic clinical form occurred in 85.6% of cases and the acute/subacute form occurred in 14.4% of cases, but most of these juvenile type cases occurred in northwestern Argentina (NWA). In NEA, the incidence of the chronic form was 90.6%; in NWA, the acute/subacute form exceeded 37%. Diagnosis by microscopy showed 96% positivity but antibody detection displays 17% of false negatives. Tuberculosis was the most frequent comorbidity, but a diverse spectrum of bacterial, fungal, viral, parasitic, and other non-infectious comorbidities was recorded. This national multicenter registry was launched in order to better understand the current status of PCM in Argentina and shows the two endemic zones with a highly diverse epidemiology.

Abstract Scedosporium/Lomentospora species are widely distributed in nature. They are gen-erally saprophytes, but can cause opportunistic infections in immunocompromised patients and occasionally in immunocompetent patients that are difficult to treat due to high lev-els of antifungal resistance. The distribution of Scedosporium/Lomentospora species shows regional differences. Scedosporium boydii and Scedosporium apiospermum are the most fre-quently isolated species in our region, whereas Scedosporium aurantiacum is more common in other regions. We describe the first isolation in Argentina of S. aurantiacum in a vitreous humor infection from a previously healthy patient after traumatic injury in her left eye. Due to the suspicion of fungal endophthalmitis, a mycological study of the vitreous humor was performed. The culture allowed the isolation of S. aurantiacum. The patient was treated with voriconazole with favorable clinic evolution.


Resumen Las especies de Scedosporium/Lamentospora se encuentran ampliamente distribuidas en la naturaleza. En general son saprofitas, pero pueden causar infecciones oportunistas de difícil tratamiento debido a sus altos niveles de resistencia a los antifúngicos en individuos inmunocomprometidos y, ocasionalmente, en personas inmunocompetentes. La distribución de las especies de Scedosporium/Lamentospora muestra diferencias regionales. Scedosporium boydii y S. apiospermum son las especies más frecuentemente aisladas en nuestra región, mientras que en otras S. aurantiacum es más común. Presentamos el primer aislamiento en Argentina de S. aurantiacum de una infección de humor vítreo de un paciente previamente sano que sufrió una lesión traumática. El paciente fue tratado con voriconazol y tuvo una evolución clínica favorable.

Resumen La esporotricosis es una infección fúngica de evolución subagudacrónica causada por hongos dimórficos del Complejo Sporothrix schenckii. Es más frecuente en zonas tropicales. La prevalencia en Argentina se estima entre 0,01 y 0,02%. En la mitad de los pacientes se manifiesta como una esporotricosis linfocutánea, la cual se produce tras la inoculación del hongo en la piel luego de un trauma menor. La lesión inicial es una pápula o nódulo que se sucede con la aparición de una cadena ascendente de nódulos subcutáneos móviles, indoloros y eritematosos. El diagnóstico se realiza a partir del cultivo micológico. El antifúngico de elección es itraconazol y el pronóstico es usualmente favorable. Se presenta el caso de una niña de 4 años, previamente sana, que consultó por adenopatías axilares de evolución subaguda sin respuesta a múltiples esquemas antimicrobianos, confirmándose el diagnóstico de una esporotricosis linfocutánea por el cultivo de una biopsia ganglionar.

ANTECEDENTES: La histoplasmosis es una micosis sistémica endémica en América con pocos casos reportados en pediatría. OBJETIVOS: Describir la epidemiología, formas clínicas y evolución de la histoplasmosis en niños de un hospital pediátrico en Argentina. MÉTODOS: El estudio, retrospectivo y descriptivo, se llevó a cabo entre enero de 2008 y diciembre de 2016 en el Hospital de Pediatría «Prof. Dr. Juan Pedro Garrahan», e incluyó pacientes menores de 18 años con cuadro clínico, serología, cultivos y/o hallazgos histológicos compatibles con histoplasmosis. Se estudiaron 13 pacientes (siete niños y seis niñas, con una mediana de edad de 9 años y rango intercuartílico de 3,4 a 13 años), de los cuales tres niños (23%) provenían de la provincia de Buenos Aires, tres de la provincia de Santa Fe (23%) y siete (54%) de otras provincias. RESULTADOS: Diez pacientes (77%) presentaron la forma diseminada de la enfermedad y tres (23%) la forma pulmonar (23%); ocho niños (62%) tenían otras comorbilidades. La serología fue positiva en siete casos (54%), los cultivos fueron positivos en nueve pacientes (69%) y en diez casos (77%) se identificaron hallazgos histológicos compatibles con histoplasmosis. Todos los pacientes recibieron anfotericina B (liposomal en siete casos y desoxicolato en seis). Diez pacientes (77%) continuaron el tratamiento con itraconazol y tres pacientes (23%) fallecieron por causas no relacionadas con la histoplasmosis. CONCLUSIONES: En la serie analizada predominaron los pacientes con histoplasmosis diseminada asociada a otra enfermedad. La mortalidad no estuvo directamente relacionada con la infección

La infección fúngica invasora ha aumentado en frecuencia a lo largo de la última década, y la sinusitis fúngica es cada vez más habitual. Los hongos del género Exserohilum (familia Pleosporaceae, orden Pleosporales) son filamentosos y dematiáceos, de localización ubicua. Se trata de patógenos emergentes, que producen, en la mayoría de los casos, infecciones sistémicas que afectan, principalmente, a los senos paranasales y los pulmones. Son más frecuentes en pacientes inmunosuprimidos, aunque pueden presentarse en pacientes inmunocompetentes. El tratamiento de estas infecciones comprende el tratamiento antifúngico, resección quirúrgica y restitución de la inmunidad. Se presenta el caso de una paciente con recaída medular de leucemia linfoblástica aguda con sinusitis fúngica invasiva por Exserohilum rostratum.

The fungi of the genus Exserohilum (family Pleosporaceae, order Pleosporales) are filamentous and dematiaceous of ubiquitous location. It is an emerging pathogen, which in most cases produces a systemic infection that mainly affects the paranasal sinuses and lungs. It is more common in immunosuppressed patients, although it may occur in immunocompetent patients. The treatment is based on three pillars: antifungal treatment, surgical debridement and restitution of immunity. We present the case of a patient with medullary relapse of acute lymphoblastic leukemia with invasive fungal sinusitis by Exserohilum rostratum.


La infección fúngica invasora ha aumentado en frecuencia a lo largo de la última década, y la sinusitis fúngica es cada vez más habitual. Los hongos del género Exserohilum (familia Pleosporaceae, orden Pleosporales) son filamentosos y dematiáceos, de localización ubicua. Se trata de patógenos emergentes, que producen, en la mayoría de los casos, infecciones sistémicas que afectan, principalmente, a los senos paranasales y los pulmones. Son más frecuentes en pacientes inmunosuprimidos, aunque pueden presentarse en pacientes inmunocompetentes. El tratamiento de estas infecciones comprende el tratamiento antifúngico, resección quirúrgica y restitución de la inmunidad. Se presenta el caso de una paciente con recaída medular de leucemia linfoblástica aguda con sinusitis fúngica invasiva por Exserohilum rostratum.

La información sobre el uso de posaconazol en niños es escasa. Se realizó este estudio descriptivo retrospectivo entre agosto de 2010 y marzo de 2017 para evaluar las características clínicas, microbiológicas y la evolución de los pacientes tratados con posaconazol. Se incluyeron 16 niños. Mediana de edad: 161 meses (rango intercuartílico -RIC- 69-173 m). Todos tenían enfermedad subyacente y presentaban infección fúngica invasiva probada. Los aislamientos más frecuentes fueron Mucor spp. y Aspergillus spp. La dosis media de posaconazol fue 600 mg/día (400-800 mg/día) y la mediana de duración del tratamiento, 223 días (RIC 48-632). Diez pacientes presentaron efectos adversos, pero solo uno requirió suspensión del antifúngico debido a alteraciones hidroelectrolíticas.


There is limited information on the use of posaconazole in children. We included 16 children. Median age: 161 months (interquartile range -IQR-69-173m). All had underlying disease and a proven invasive fungal infection. The most frequent isolated were Mucor spp. and Aspergillus spp. The mean posaconazole dose was 600 mg /day (400-800 mg/day) and the median duration of treatment was 223 days (IQR 48-632). Ten patients had adverse effects, but only one required suspension of the antifungal treatment due to hydroelectrolytic disorders.

There is limited information on the use of posaconazole in children. This retrospective and descriptive study was conducted to evaluate the clinical, microbiological characteristics and evolution of patients treated with posaconazole between August 2010 and March 2017. Se realizó este estudio descriptivo retrospectivo entre agosto de 2010 y marzo de 2017 para evaluar las características clínicas, microbiológicas y la evolución de los pacientes tratados con posaconazol. Se incluyeron 16 niños. Mediana de edad: 161 meses (rango intercuartílico ­RIC­ 69-173 m). Todos tenían enfermedad subyacente y presentaban infección fúngica invasiva probada. Los aislamientos más frecuentes fueron Mucor spp. y Aspergillus spp. La dosis media de posaconazol fue 600 mg/ día (400-800 mg/día) y la mediana de duración del tratamiento, 223 días (RIC 48-632). Diez pacientes presentaron efectos adversos, pero solo uno requirió suspensión del antifúngico debido a alteraciones hidroelectrolíticas.

La osteomielitis en pacientes quemados es infrecuente. Objetivo: Describir las características clínicas, microbiológicas y de evolución de niños quemados con osteomielitis internados en un hospital de alta complejidad. Objetivo. Describir las características clínicas, microbiológicas y evolutivas de niños con diagnóstico de osteomielitis internados en una Unidad de Quemados Pediátrica de alta complejidad. Métodos: Estudio retrospectivo y descriptivo realizado durante el período de enero de 2007 a enero de 2017. Resultados: Sobre un total de 600 niños quemados, 12 presentaron osteomielitis (incidencia del 2%). Once pacientes presentaron quemadura por fuego directo. La mediana de edad fue 42,5 meses (rango intercuartílico -RIC- 27-118 meses) y de superficie quemada fue 33,5% (RIC 18,5-58%). La osteomielitis se diagnosticó con una mediana de 30 días posquemadura. Las localizaciones más frecuentes fueron las extremidades superiores y la calota. La fiebre fue la manifestación clínica más común. Los microorganismos más frecuentemente aislados en el tejido óseo fueron hongos en 9 pacientes. Todos presentaron anatomía patológica compatible. La mediana de tratamiento fue 44,5 días (RIC 34,5-65,5 días). Seis pacientes presentaron secuelas motoras y 1 paciente falleció. Conclusión: La etiología fúngica fue la más frecuente. La mitad de los pacientes presentó secuelas funcionales y solo un paciente falleció.

The treatment lasted a median of 44.5 days (IQR: 34.5-65.5 days). Six patients had motor sequelae and 1 died. Conclusion: Fungal osteomyelitis was the most commonly observed etiology. Half of patients had functional sequelae and only 1 patient died.

Trichosporon asahii es un hongo ubicuo que se ha aislado como parte de la microbiota humana. Recientemente, se ha visto una emergencia de este patógeno en infecciones tanto localizadas como sistémicas. En unidades de cuidados intensivos pediátricos para quemados, existen escasos reportes de infecciones del tracto urinario por este microorganismo. Se describen 2 pacientes pediátricos con internación prolongada por quemaduras extensas y múltiples tratamientos antibióticos previos. Ambos presentaron sepsis por infección del tracto urinario asociada a sonda vesical por Trichosporon asahii. En ambos pacientes, se realizó el recambio de la sonda vesical y tratamiento con voriconazol por 10 días, con buena evolución. En los casos presentados, debido a la ausencia de otros aislamientos microbiológicos y a la buena respuesta al tratamiento antifúngico junto con el recambio de la sonda vesical, se asumió al Trichosporon asahii como el probable agente causal de la sepsis.

La experiencia con anidulafungina en el tratamiento de infecciones fúngicas invasivas en pediatría es escasa. Se presenta nuestra experiencia en 55 niños. La anidulafungina se administró por vía intravenosa en la dosis de carga de 3 mg/kg en una sola dosis diaria, seguida de 1,5 mg/kg cada 24 h durante una media de 14 días (rango, 7-22 d.). La mediana de edad de los pacientes fue de 114 meses (rango intercuartíhco, 32168 m.). Todos los pacientes tenían enfermedades subyacentes. En los trasplantados de médula ósea, la diferencia entre el valor inicial y al final de la administración del fármaco en el recuento de glóbulos blancos, valores de transaminasas y función renal no fue significativo. Ninguno de los pacientes tuvo eventos adversos o murió por causas relacionadas con anidulafungina. La anidulafungina podría ser una opción para la profilaxis o el tratamiento de las infecciones fúngicas invasivas en pediatría, aunque se requieren estudios metodológicamente sólidos para probarlo.
